For me, it's when someone who is usually a hard working and genuinely nice person suddenly starts acting like a lazy jerk.
Like not even doing anything outwardly harmful but just sort of being inconsiderate and insensitive whenever you try to talk to them. Like if you ask them what's wrong they might reply with "what's wrong is your face you ugly fucking dumbass!" Like they won't go out of their way to harm you but if you talk to them they'll reply rudely.
Or someone who's usually clean and neat leaving trash all over the floor at work or at school. Or neglecting their hygiene, not changing their clothes, smelling bad, etc. Or someone who normally contributes a lot to group projects suddenly contributing nothing.
And during all this time they just have a sort of dead, lifeless look in their eyes. Like they're not really enjoying acting like this, but they're not getting the help they need from anywhere and they're just lashing out because they're desperate.
In these cases, not much you can do to help them as you are most likely not a therapist, psychiatrist, or psychologist. But you can try talking to them and inviting them to stuff... if they react negatively though, probably best to leave them be till they figure it out for themselves.
