As someone who has bimax-maxxed, I would argue that getting surgery means you care more about self development and less about others opinions than a normie does.
You can argue that people get surgery because they want to be perceived better by other people, but for me first and foremost is was about feeling complete and revealing my potential and what I believed to be my "true self"
I didn't try track down any old friends afterwards to see if they were impressed by my transformation.
Even though it's nice to get more positive feedback afterwards, if I just ended up rotting in the woods afterwards it would've been worth it to me because inherently I only care about my own progress and development rather than other's perception of it
Whereas you take the standard normie, he doesn't consider feeling fully comfortable in himself to be primary goal, rather he sees the validation he can receive from others by displaying status symbols (job, car, watch) to be the primary goal because ultimately he cares more about attaining things he believes will raise other people's opinion of him than he cares about feeling like he is completing himself or reaching self actualization.
Caring more about things that will impress others than things that will impress yourself indicates insecurity
