I think he was riding his bike when the black dude did something to him, breaking one of his teeth at the gum line (where the tooth meets the gums)
Something similar happened to my father on two occasions, the first was when two guys attacked him to steal the bicycle he was using, and the second was when a car crossed over him and he fell injured.
